Job summary

Enhabit Home Health & Hospice is seeking a dedicated Physical Therapist to deliver patient-centered care in the home setting. You will assess impairments, design treatment plans, and apply modalities to improve mobility and independence in daily living.

Responsibilities include administering skilled services per physician orders, educating patients and families, and collaborating with a multidisciplinary team under the Branch Director's supervision.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments to improve mobility and function.
• Teach and guide patients and families in self-care and home exercise programs under physician orders.
