Job summary

CenterWell Home Health seeks a Home Health Physical Therapist to deliver in-home PT, assess patient function, and develop rehab programs. You will work with a care team to meet patient goals, documenting progress and communicating with the physician and family.

Reporting to the Clinical Manager, you will support a multidisciplinary approach in a home-based setting. The role includes coordination with PTAs and Home Health Aides, providing clear care instructions, and maintaining thorough

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy treatment in patients' homes to attain highest level of physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program aligned with physician's Plan of Treatment and patient goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to physician, Clinical Manager, care team, patient and family; submit evaluations, plans, progress notes, and discharge summaries.
