Job summary

Elara Caring is seeking a Physical Therapist to deliver high-quality home-based care across Illinois. You will provide direct patient care, evaluate needs, and adjust plans to achieve optimal outcomes for patients in their homes.

You will report to Branch Director or Clinical Manager, travel up to 50%, and lift up to 50 pounds. A $15,000 sign-on bonus and comprehensive benefits accompany this role.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Able and willing to travel within branch/office coverage area (50%).
• Must have a dependable vehicle, valid driver’s license, and auto insurance as per state laws.

Responsibilities

• Provides direct patient care and evaluates functional needs and outcomes.
• Reviews and adjusts care plans to achieve optimal patient outcomes.
• Documents evaluations, treatment goals, and plans; maintains signatures.
