Job summary

The Legacy Senior Communities is seeking a Licensed Physical Therapist to provide intermittent home health services in clients’ homes in Plano, TX. The role requires evaluating patients, planning treatments, coordinating with physicians and the care team, and educating patients and families.

Applicants must have a PTA/OT license, Texas PT license, CPR certification, and at least one year of institutional experience.

Qualifications

• Graduate of a physical therapy school approved by the AMA Council and APTA.
• Current license in the state of Texas.
• Current CPR certification.
• Has at least one year of institutional experience.

Responsibilities

• Initial and interim evaluation.
• Problem identification and goal setting.
• Treatment planning in consultation with the physician to achieve goals.
• Interdisciplinary coordination.
• Client/family education as appropriate.
• Provision of equipment to increase function and independence.
• Timely recording of evaluation data and patient response.
• Discharge planning and documentation.
