Job summary

Elite Home Health, part of the LHC Group and within the Optum family, seeks a Physical Therapist to evaluate patient needs in home health and develop treatment plans. You will implement interventions to improve well-being and assess progress in a dynamic, patient-centered setting.

Responsibilities include evaluating functional status, designing care plans, coordinating with the health care team, documenting visits, and educating patients and families about goals and plan of care.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs as part of plan of care development.
• Provides services within the scope of practice and coordinates with the health care team.
• Observes and reports patient responses, and adjusts treatment accordingly.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Educates patient and family regarding plan of care and progress toward goals.
• Visits patients per Plan of Treatment and documents progress notes.
• Completes evaluations and develops PT plan of care within state guidelines.
• Reports outcomes and frequency of care.
• Function effectively in diverse home environments.
