Job summary

Enhabit Home Health & Hospice is seeking a Physical Therapist in Des Plaines, IL to evaluate and treat patients with mobility challenges, helping them regain independence and daily function. You will work within established care plans and coordinate with physicians and family members to ensure continuity of care.

The role requires driving to patient homes, valid state licensure, and commitment to high-quality home health services.

Qualifications

• Must possess a valid state driver's license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be currently licensed in the state of employment.

Responsibilities

• Therapists examine and treat patients with physical impairments to improve mobility and self-care.
• Administer skilled care under physician orders and plan of care, under supervision of Branch Director.
• Educate and involve family and health care team members in care plans.
