Qualifications

• Graduate from an accredited physical therapy program with a Florida license.
• Minimum of one year of PT experience, preferably in home health.
• Ability to create effective physical therapy treatment plans that promote recovery.
• Strong communication with patients, families, and other healthcare professionals.
• Ability to work independently and collaboratively as part of a larger healthcare team.
• Ability to read and interpret physician orders and implement treatment accordingly.

Responsibilities

• Conduct comprehensive physical therapy evaluations and create personalized treatment plans to help patients achieve their goals.
• Teach patients and families how to properly perform exercises to promote recovery and improve mobility.
• Document patient progress and communicate updates to other healthcare professionals.
• Coordinate care with physicians, nurses, social workers, and speech therapists to ensure effective treatment.
• Adhere to policies and infection control and safety protocols.
• Educate patients and families on injury prevention and self-care techniques.
• Maintain accurate records of patient interactions and treatment plans in compliance with regulations.
