Job summary

Enhabit Home Health & Hospice is seeking a Physical Therapist to evaluate and treat patients in home-based settings, focusing on improving mobility and independence. You will administer skilled services per physician orders and collaborate with a multidisciplinary team under branch leadership.

You will educate patients and families, participate in specialty programs, and ensure care aligns with the plan of care and safety standards across weather and transportation considerations.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities to improve mobility and function.
• Provide skilled care to clients per physician orders and plan of care, under supervision of the Branch Director.
• Educate family and other health care team members as part of the care plan.
• Participate in specialty programs such as Balance and Fall Prevention, Spine Safety, and Hip/Knee Replacement initiatives.
