SHC WVUHS Home Care is seeking an Occupational Therapist to evaluate, plan, and progress OT interventions during home visits. The role includes medical record completion, insurance forms, and supervising Rehab associates.

You will collaborate with the care team, educate patients on home programs and devices, and ensure Medicare guidelines are followed for SOC, recertification, and discharge.

Qualificações

• Current Occupational Therapist licensure as required by the state of service.
• Valid driver’s license.
• CPR certification within 30 days of hire.

Responsabilidades

• Evaluate and implement goal-directed OT interventions.
• Consult with medical staff and health professionals.
• Complete medical records and insurance forms.
• Supervise Rehab associates.
• Educate patients on home programs and assistive devices.
• Coordinate care team for level-of-care changes.
• Complete SOC, recertification, and discharge per Medicare guidelines.
• Perform CPR as directed by patient wishes.
