Job summary

Mission Healthcare is seeking a licensed Occupational Therapist to evaluate patients in their home settings within the West Valley territory (Goodyear, Glendale, Surprise, Avondale, and surrounding areas). You will develop personalized plans, implement rehabilitation activities, and educate families to promote independence.

The role requires a renewal license, BLS/CPR, and a reliable vehicle. Schedule is Mon-Fri 8am-5pm, with competitive benefits and relocation options depending on location.

Qualifications

• Graduate from an accredited Occupational Therapy program.
• Valid state OT license.
• Minimum of 1 year of experience practicing as an Occupational Therapist.
• Current BLS and CPR certification.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Evaluate patients’ functional abilities in their home environment and develop personalized treatment plans.
• Implement therapeutic activities to improve daily living tasks.
• Educate patients and caregivers on techniques and adaptive equipment to promote safety and independence.
• Monitor progress and update care plans based on outcomes.
• Document visits and coordinate with the home health interdisciplinary team.
