AdvisaCare is hiring a registered nurse for full-time home health in Grand Rapids, Michigan. You will perform thorough patient assessments, develop care plans, administer medications, and educate patients and families in their homes.

Qualifications

• Active Registered Nurse license in the state of practice.
• Minimum 1 year home health experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Valid driver's license and reliable transportation.
• Ability to work independently and manage caseload.

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans.
• Administer medications and treatments as prescribed.
• Provide skilled nursing care and education to patients and families.
• Ensure compliance with safety, infection control, and documentation standards.
• Collaborate with interdisciplinary teams to coordinate patient care.
