Enhabit Home Health & Hospice is seeking a qualified nurse to provide skilled nursing care in patients’ homes, following the physician-approved plan of care. You will educate patients and families, coordinating with the health care team to ensure high-quality outcomes.

The role requires active state licensure for a registered nurse, at least one year of nursing experience, and strong comfort with mobile technology for documentation and communication.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed nurse.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled nursing services to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
