Job summary

Elara Caring is seeking a Speech-Language Pathologist to join a team delivering high-quality care in patients' homes across Indiana. The role involves evaluating speech-language needs, developing plans of care, and coordinating with physicians and family members to ensure continuity of care.

Qualifications

• Master’s degree in Language Pathology required.
• Current, unrestricted SLP license in state of work.
• Minimum one year of experience as an SLP in a clinical care setting.

Responsibilities

• Promotes Elara Caring’s mission and policies to ensure quality of care.
• Evaluates patients and develops a plan of care.
• Admits patients, assesses needs and conducts initial and follow-up evaluations per regulatory guidelines.
• Communicates findings to supervisors and team members.
• Participates in Plan of Care implementation and discharge planning.
• Performs direct Speech-Language pathology services per standards of practice.
• Instructs patients and caregivers in home therapy programs.
• Maintains accurate therapy notes and logs.
