Job summary

Claratel Behavioral Health is seeking a Forensic Peer Mentor to support release planning for individuals in DeKalb County jail with mental health and co-occurring conditions. Qualifications

• High school diploma or equivalent required; associate degree preferred.
• Experience with local behavioral health, social services, and criminal justice systems.
• Care coordination skills to link individuals to resources.
• Ability to work in recovery-focused, trauma-informed model.
• Must have valid Georgia driver’s license and transportation.

Responsibilities

• Provides Community Transition Planning according to guidelines.
• Completes the Community Transition Plan with individuals in the DeKalb County Jail.
• Provides screening to support access to necessary services (mental health, substance use, etc.).
• Provides referral and linkage to services to reduce hospitalization, incarceration, and homelessness.
• Arranges outpatient follow-up services at DeKalb CSB or other providers as needed.
• Maintains documentation per agency standards.
• Attends required meetings and coordinates with community providers.
