Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, Counseling, or related field (required).
• Master’s degree preferred.
• Minimum of one year of case management/family support experience.
• Bilingual candidates encouraged to apply.
• Valid driver's license and reliable transportation required.

Responsibilities

• Conduct weekly home visits with families, including evenings as needed.
• Provide family-centered support, advocacy, resource coordination, and skill-building services.
• Document case notes and home visits in Credible; complete monthly reports and treatment plans.
