Youth Advocate Programs, Inc. is seeking an Intensive Family Coordinator to provide professional case management and direct advocacy to a caseload of families in Michigan. You will assess families and develop individualized service plans, coordinate resources, and document activities in compliance with state guidelines.

The role requires travel, occasional nights and weekends, and availability for 24/7 crisis support. Strong communication, organization, and computer skills are essential.

Qualifications

• High School Diploma or GED required.
• Experience in human services fields is required.
• Experience in home and community-based services preferred.
• Proficiency in MS Office (Word, Excel, Outlook).
• Knowledge of Electronic Health Record (EHR) systems is a plus.
• Strong written and verbal communication skills.
• Exceptional organization and attention to detail.
• Reliable transportation, valid driver’s license, and current auto insurance.

Responsibilities

• Provide professional case management services to a caseload of families.
• Complete family assessments and create individualized service plans.
• Recruit foster parents and complete home studies as needed.
• Coordinate resources and document case activity per state regulations.
• Travel as needed and be available for nights, weekends, and 24/7 crisis support.
