Prince William County Community Services in Woodbridge, VA seeks a Service Coordinator III (Senior Clinical Services Caseworker) to supervise up to 4 staff and provide advanced case management for individuals with developmental disabilities. The role includes intake, eligibility, needs assessment, and service plan development with coordination of home and community services.

The ideal candidate will have a bachelor's degree in a behavioral science field, at least 3 years of related experience,

Qualifications

• Bachelor's degree in psychology, social work, counseling, or related behavioral science.
• 3 years of experience providing service coordination and case management to individuals with developmental disabilities.
• Knowledge of person-centered practices, developmental disabilities, and dual diagnoses (DD/MH).
• Experience using electronic health records; Credible preferred.
• Experience using WaMS for DD Waiver recipients.

Responsibilities

• Supervise up to 4 staff and manage a caseload of individuals needing services from CS or other programs.
• Conduct intakes, eligibility determinations, needs assessments, and service plan development.
• Coordinate and lead treatment team meetings for service planning and reviews.
• Implement and revise individual service plans; monitor quality of care through visits.
• Document all services and contacts; write progress notes and quarterly reviews.
• Maintain trainings and provide in-service trainings; ensure regulatory compliance.
• Organize interdisciplinary team meetings and manage service authorizations and vendor approvals.
