Job summary

Elara Caring in Illinois is seeking a Certified Occupational Therapist Assistant to continue delivering high-quality in-home care. You will follow care plans, monitor progress, and work with physicians and the care team to ensure coordinated treatment for patients.

The role requires a current COTA credential, at least one year of experience, ability to lift up to 50 pounds, and a valid driver's license or reliable transportation to travel between patient homes.

Qualifications

• Graduate of a certified occupational therapy curriculum established by the Occupational Therapy Association.
• Current, unrestricted COTA certification in the state of work.
• Minimum one year of experience as a COTA in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds including positioning or transferring patients and moving equipment.

Responsibilities

• Provide occupational therapy services by following the care plan developed by the Occupational Therapist.
• Visit patients regularly and monitor their progress during each visit.
• Report changes in a patient's condition to the Occupational Therapist, physician, and care team.
• Follow physician orders and the established plan of care.
• Teach patients and caregivers home therapy exercises and safe techniques.
• Work closely with the care team to provide coordinated, high-quality patient care.
• Complete accurate and timely documentation for each patient visit.
• Help reduce avoidable hospital and emergency room visits through proactive patient care.
• Stay up to date on healthcare regulations and agency policies.
