Elara Caring in Indiana seeks a Certified Occupational Therapy Assistant to provide hands-on OT services per the plan, visit patients, monitor progress, and collaborate with the care team to deliver high-quality, compassionate home-based care.

You will document visits accurately, educate patients and caregivers on home exercises, and help reduce unnecessary hospital visits while staying current with regulations.

Qualifications

• Graduate of a certified OT curriculum established by the Occupational Therapy Association.
• Current, unrestricted COTA certification in the state of work.
• Minimum one year of experience as a COTA in a clinical care setting.

Responsabilités

• Provide OT services per the plan developed by the Occupational Therapist.
• Visit patients regularly and monitor their progress.
• Report changes in a patient's condition to the OT, physician, and care team.
• Follow physician orders and the established plan of care.
• Teach patients and caregivers home therapy exercises and safe techniques.
• Work closely with the care team to provide coordinated, high-quality patient care.
• Complete accurate and timely documentation for each patient visit.
• Help reduce avoidable hospital and emergency room visits through proactive patient care.
• Stay up to date on healthcare regulations and agency policies.
• Work in a physically active role that may include lifting, transferring patients, and occasional irregular or on-call hours.
