Job summary

Virginia Commonwealth University Health Systems seeks an Advanced Practice Provider (APP) to deliver primary care for patients with multiple chronic conditions in a collaborative, team-based clinic. The APP will work with social workers, pharmacists, and RN case managers to coordinate care across acute and chronic needs.

The role requires closing with Attending Physicians, performing histories and exams, ordering tests, and prescribing medications as needed within the defined scope of practice.

Qualifications

• Graduation from an accredited Nurse Practitioner, Nurse Midwife or Physician Assistant Program.
• Current Virginia licensure as a Nurse Practitioner, Nurse Midwife, or Physician Assistant
• Current national certification appropriate for the role
• Current CPR and BLS certifications; ACLS/PALS may be required at department level

Responsibilities

• Provides primary care to patients in a team-based care setting.
• Collaborates with Attending Physician and multidisciplinary team to coordinate care.
• Performs histories, physicals, orders/ interprets tests, and prescribes medications as appropriate.
• Assists with surgical procedures and provides patient education and counseling.
