Northern Counties Health Care, Inc. seeks an Advanced Practice Provider to deliver comprehensive primary care to diverse, underserved populations in Vermont.

You will diagnose, treat, educate, and manage patients within scope, while maintaining confidential, high-quality care in a team-based setting. Responsibilities include performing exams, ordering tests, reviewing results, and coordinating referrals with specialists.

Qualifications

• Graduate of an accredited Physician Assistant, Family Nurse Practitioner, Adult NP, or Pediatric NP program.
• Licensed in the State of Vermont.
• Certified by a relevant National Certification Examination.
• DEA License appropriate for scope of practice as defined by State law and clinic protocols.

Responsibilities

• Provide primary care, treatment and health education for patients.
• Document assessment, planning, implementation and evaluation of patient care.
• Perform or order laboratory tests per clinic protocols and CLIA guidelines.
• Review test results and follow up on abnormal findings.
• Refer patients for specialty care and coordinate ongoing medical review.
