Qualifications

• High School Diploma or equivalent is required.
• Strong communication and interpersonal skills are essential.
• Experience with case management services is preferred.
• Proficiency with Microsoft Office is required.
• Ability to obtain QMHS certification within 60 days of hire.

Responsabilités

• Develop and implement individualized care plans with measurable outcomes.
• Maintain documentation including intake, care plans, case notes, and data entry.
• Provide case management services including assessment, referrals, and follow-up.
• Coordinate with school staff, families, and external providers to support students.
