Job summary

ChristianaCare seeks a Clinical Pharmacist to join the Newark Campus team. This full-time role supports daily interdisciplinary rounds, manages complex therapies, and monitors patient outcomes within a decentralized acute care model.

Responsibilities include drug information provision, consultations, and participation in quality improvement activities. Delaware licensure is required, with opportunities to precept students and residents.

Qualifications

• Bachelor of Science in Pharmacy or PharmD required.
• ASHP-accredited residency preferred or equivalent acute care experience.
• Board certification preferred.
• Current licensure in Delaware or eligibility to reciprocate.

Responsibilities

• Participate in direct patient care through daily interdisciplinary rounds.
• Manage specific medication therapies per approved policies (e.g., warfarin, vancomycin, dose adjustments).
• Evaluate drug efficacy and monitor therapeutic goals.
• Order and review laboratory tests to support therapy monitoring.
• Counsel and obtain medication histories; communicate recommendations to providers.
