Job summary

Lutheran Services Florida seeks a dedicated Case Manager to provide wrap-around case management for children and families, building strong relationships aligned with the agency’s values.

Responsibilities include developing family-centered case plans, coordinating with guardians and service providers, and ensuring timely documentation in FSFN while adhering to state regulations and safety protocols.

Qualifications

• Excellent written and verbal communication skills.
• Familiarity with and ability to use Microsoft Office programs Word and Excel.
• Ability to drive both locally and throughout the state in connection with the duties of this position.
• Must demonstrate sensitivity to our service population’s cultural and socioeconomic characteristics and needs.

Responsibilities

• Accept all cases assigned by unit supervisor and meet with children in their primary residence within 2 working days of staffing.
• Negotiate and develop a case plan with parents, Guardian Ad Litem, foster parents and other parties toward reunification or permanency.
• Visit the child in their primary residence at required frequency, with unannounced visits quarterly.
• Document all case activities in FSFN within 48 hours and maintain files.
• Submit service requests and refer clients to services; maintain knowledge of community resources.
