Valley Hope of Moundridge is seeking a Case Manager to join our compassionate team in a residential treatment setting just north of Wichita. You will support continuing care, facilitate recovery group sessions, and help patients access housing, education, and medical resources.

Qualifications

• 1+ year of experience in a similar position or healthcare environment.
• CPR certification; or ability to obtain within 30 days of hire.

Responsibilities

• Plans and implements continuing care for assigned patients.
• Facilitates rehabilitation group sessions and workshops on recovery topics.
• Identifies recovery resources for housing, education, vocational development, mental health, and medical care.
