Job summary

The Pennant Group, Inc. is seeking an Advantage Case Manager to perform admission/intake, assessments, services plan implementation, monitoring/evaluation, and discharge of long-term care clients under OKDHS standards, under supervision of CMS and/or Excell Private Care Services management team.

This role emphasizes advocacy, coordination of community resources, documentation, confidentiality, and compliance with HIPAA; requires driving, valid license, and fluent English for effective

Qualifications

• RN or LPN with professional experience in aging or disabled population.
• One year of paid professional experience preferred in case management.
• Licensed driver with insured vehicle and reliable transportation.

Responsibilities

• Establish intake, screening, and referral procedures for information requests and eligibility.
• Review assessments to identify member needs and develop service plans with IDT input.
• Educate and empower consumers to be informed and active participants in their care.
• Coordinate community resources and supports based on member needs and eligibility.
• Submit comprehensive service plans and monitor delivery of services.
• Identify and address high-risk situations; maintain HIPAA-compliant records.
• Communicate with medical personnel regarding medically complex cases and ensure confidentiality.
