Qualifications

• Six months of experience in human services, direct care, or care coordination preferred.
• High School Diploma or equivalent required.
• Valid driver's license with access to an insured vehicle.

Responsibilities

• Assist participants with personal hygiene and daily living activities.
• Support participants in community integration and activities.
• Build trusting relationships with individuals and families.
• Document progress and communicate with supervisors.
