Northern Pines Mental Health Center is seeking a Care Coordinator based in Brainerd, MN to support clients across multiple services. The role largely involves coordination, linkage to primary care, and resource navigation across service locations.

In this full-time position, you will participate in treatment plan meetings, provide crisis resources, and collaborate with community partners to ensure clients receive the necessary care and referrals. A valid driver's license is required.

Qualifications

• High School Diploma or approved equivalent combination of education and experience.
• Travel is a function of this position; a valid driver's license is required.

Responsabilités

• Attend Integrated Treatment Plan meetings for clients enrolled in Care Coordination.
• Attend Rule 79 Case Consultation Meetings to facilitate client needs.
• Attend Thursday Integrated Consultation Meetings.
• Assess clients' needs to establish primary care, dental care and eye care with linkage and follow-up.
• Provide SDoH resources and referrals to meet client needs.
• Close the loop on resources and referrals to ensure client engagement.
• Submit referrals after client discussion with Mental Health Professional.
• Advocate for clients to help ensure their needs are met.
• Assess client satisfaction with services.
• Provide MNSURE navigation for uninsured individuals.
• Provide crisis resources and assist in connecting to crisis services.
• Provide health and wellness education to clients.
• Schedule and attend ongoing visits for Care Coordination program.
• Collaborate with community partners; may include travel to community sites.
• Manage daily reports in EHR and workflow folders.
• Maintain accurate documentation of services provided.
• Outreach to clients regarding alerts from EAS about admissions/discharges.
