Job summary

SnapCare is seeking a Physical Therapist for a 13-week assignment in Willingboro, NJ. Clinic operates Monday through Friday, 8:00 AM to 4:30 PM, with no weekend shifts.

You will evaluate patients, develop and modify treatment plans, and collaborate with an interdisciplinary team to facilitate discharge planning and community reintegration. The role requires a valid state license and BLS certification prior to the first shift, plus a minimum of two years of experience, with acute care or LTACH

Qualifications

• Requires minimum 2 years of experience
• BLS certification required prior to first shift
• Valid, current State license required
• Acute care or LTACH experience encouraged

Responsibilities

• Evaluate assigned patients and develop, implement, and modify treatment plans in accordance with patient needs and physician direction
• Delegate and direct all discipline-specific therapy services for assigned patients
• Design functional treatment programs to support return to community activities
• Collaborate with interdisciplinary team to address barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services
• Complete medical record documentation including daily notes, goal writing, functional assessments, letters of medical necessity, discharge documentation, and charge entry
• Manage individual, concurrent, and group treatment sessions
• Participate in student program as assigned, providing supervision and evaluations
• Maintain licensure and complete state-mandated continuing education requirements
