Responsibilities

Assesses appropriateness of each medication based on diagnosis, condition, allergies, and history; reviews for adverse drug reactions and drug interactions; promotes rational therapy and makes recommendations to physicians.
Monitors patient response to prescribed therapies; reevaluates therapy and makes recommendations for changes when appropriate.
Documents patient medication history and notifies physicians or healthcare professionals of compliance or therapy issues.
Ensures responsible distribution of medications in the assigned area and triages orders to meet immediate patient care needs.
Provides drug information to patients, families, physicians, nurses and other health care professionals; shares new literature to optimize medication use.
Supports educational programs by precepting students and residents.
