Responsibilities

• Assesses appropriateness of each medication based on diagnosis, condition, allergies, and history; reviews for adverse drug reactions and drug interactions; promotes rational therapy and makes recommendations to physicians.
• Monitors patient response to prescribed therapies; reevaluates therapy and makes recommendations for changes when appropriate.
• Documents patient medication history and notifies physicians or healthcare professionals of compliance or therapy issues.
• Ensures responsible distribution of medications in the assigned area and triages orders to meet immediate patient care needs.
• Provides drug information to patients, families, physicians, nurses and other health care professionals; shares new literature to optimize medication use.
• Supports educational programs by precepting students and residents.
