Qualifications

• Associate degree or two years of social/recreational program experience in last five years.
• NCAP certification or enrollment in approved activities training course.
• Completion of a state-approved activities training course.

Responsibilities

• Assists in planning, conducting and evaluating scheduled activities for patients.
• Leads group activities and maintains attendance records.
• Collects oral histories and completes patient assessments, progress notes and care plans under director.
