Job summary

Community Housing Innovations, Inc. (CHI) is seeking a full-time Case Manager at our Medford location to help participants achieve independent housing and economic stability. You will guide clients toward housing readiness, develop Independent Living Plans, and connect them with necessary services.

Responsibilities include face-to-face client meetings, budgeting support, information referrals, and advocacy. Education includes a BA in human services; MSW preferred.

Qualifications

• Excellent verbal and written communication skills.
• Proficiency in English required; bi-lingual (Spanish) is a plus.
• Basic competency in MS Windows, MS Office, and internet usage.
• Ability to formulate simple financial budgets.

Responsibilities

• Adhere to program regulations, policies, and procedures.
• Assist clients in developing an Independent Living Plan (ILP).
• Provide information, referrals, counseling, crisis intervention, and advocacy.
• Develop and maintain a resource network for referrals; collaborate with DSS.
• Assist with client budgets, resumes, ADL skills, etc.
• Conduct face-to-face meetings with clients.
• Inspect living spaces to ensure program compliance and housing upkeep.
• Submit internal/external reports timely and accurately.
