Sevita is seeking a part-time Direct Support Program Coordinator to empower individuals to live more independently. You will develop and implement tailored treatments and collaborate with a treatment team to build effective service plans.

The role involves leading staff, monitoring programs, and ensuring high-quality care for those served. A driver’s license and reliable transportation are required where applicable.

Qualifications

• High School diploma or equivalent; Bachelor’s degree if required by state.
• Three years of related work experience.
• Valid driver's license in good standing and car registration/auto insurance (if driving for company).
• A reliable, responsible attitude and a compassionate approach.

Responsibilities

• Assist in accessing medical and health-related services for individuals.
• Assess referred individuals and participate in service-planning with the team and funding source.
• Implement and contribute to development, review, and revision of programs and behavior plans.
• Provide monitoring, consultation, and support to Mentors and staff.
