Job summary

Family & Children’s Services in Tulsa, OK seeks a Case Manager to liaison with criminal justice-involved clients and coordinate access to mental health and substance abuse treatment. You will work with a team including judges, law enforcement, and therapists to navigate court systems and support clients toward community reintegration.

Responsibilities include assessments, care planning, advocacy, and ensuring compliance with policies.

Qualifications

  • Bachelor’s Degree in the social sciences or related field.
  • Case Management II certification preferred or obtainable after hire.
  • Must have a strong commitment to helping clients live in community settings.
  • Valid Oklahoma Driver License and driving record to travel.
  • Strong organizational and communication skills; team-oriented.

Responsibilities

  • Conduct comprehensive assessments of clients' physical, psychological, social, and environmental needs.
  • Implement individualized care plans with clients and families to promote reintegration into the community.
  • Coordinate and monitor the delivery of services for timely and appropriate care.
  • Advocate for clients' rights and access to resources.
  • Provide emotional and practical support to clients and families.
  • Regularly evaluate progress and adjust care plans as needed.
  • Maintain accurate documentation of care, progress, and outcomes.
  • Facilitate communication among multidisciplinary teams.
  • Participate in case conferences and team meetings.
  • Educate clients and families about available services and supports.
  • Ensure compliance with laws, regulations, and standards of practice.
  • Engage in continuing education and trainings.
