Job description

The RN Case Manager is responsible for coordinating patient care throughout their hospital stay, ensuring timely and efficient service. They collaborate with multidisciplinary teams, including physicians, social workers, and healthcare professionals, to meet patients' medical, social, and emotional needs. The RN Case Manager aims to optimize resource use, improve patient outcomes, and prevent unnecessary readmissions.

Qualifications
  1. Education: Graduate from an accredited nursing school; Bachelor’s Degree preferred.
  2. Experience: At least three years in case management or utilization management preferred. Must be a Registered Nurse with a minimum of one year in medical/surgical nursing, possessing excellent communication skills and the ability to work collaboratively. Physical stamina and emotional stability are required.
  3. Licenses/Certificates: Current Illinois RN License.
Compensation and Benefits
  • Hourly Range: $31.25 - $46.88
  • Incentives: Evening/night and weekend shift incentives, based on company policies and applicable job codes. Additional details provided during the interview.
