RHA Health Services, LLC is hiring a Licensed Practical Nurse (IDD LPN) for our Rome, GA location. This full-time, onsite role supports individuals with intellectual and developmental disabilities in a group home setting. Typical schedule is days, with rotating shifts as needed, and a max caseload of 4.

• Current LPN license with no restrictions to practice nursing.
• Current CPR Certification or eligibility to certify for CPR.
• Possess credentials required by Federal or State law.
• Effective communication with Interdisciplinary Team Members, as well as people supported and families.
• Ability to work independently, be organized, and prioritize work tasks.
• Must be at least 18 years of age.

Responsibilities

• Provide holistic nursing care to individuals with intellectual and developmental disabilities.
• Observe, assist with gathering health data, and report health needs to the registered nurse.
• Coordinate preventative and routine medical care for people supported.
• Document nursing care provided.
• Assist physicians with on-site medical clinics where applicable.
• Provide health-related trainings to unlicensed personnel according to state regulations.
• Conduct medication pass observations to ensure staff competency according to state regulations.
• Conduct medication closest assessments.
• Perform quality assurance checks to ensure unlicensed personnel perform health tasks as assigned.
• Observe and report abnormal health findings to the physician.
• Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team.
• Advocate for the people supported and help develop community supports for better health.

We are hiring for: Licensed Practical Nurse (IDD LPN) - Days Type: Regular

Location: Rome, GA (onsite, group home setting)

Employment Type: Full-time

Compensation: $32/hr

Incentives: No sign on bonus. No relocation packaged offered.

Schedule: 7am - 7pm, rotating weekends off

Max Patient Caseload: 4 total

Job Responsibilities

• Provide holistic nursing care to individuals with intellectual and developmental disabilities
• Observe, assist with gathering health related data, and report the health needs or the people we support to the registered nurse
• Coordinate preventative and routine medical care for people supported
• Document nursing care provided
• Assist physicians with on-site medical clinics where applicable
• Provide health related trainings to unlicensed personnel according to state regulations
• Conduct medication pass observations in the home to ensure staff competency according to state regulations
• Conduct medication closest assessments
• Perform quality assurance checks and observations to ensure unlicensed personnel are performing health related tasks as assigned according to state regulations
• Observe for and report any abnormal health related findings to the physician
• Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team
• Advocate for the people supported and assist with developing community supports to promote optimal health for the people supported

Minimum Qualifications

• Graduation from an accredited licensed nurse program.
• Current LPN license with no restrictions to practice nursing.
• Current CPR Certification or eligibility to certify for CPR.
• Possess credentials required by Federal or State law
• Must be able to communicate effectively with Interdisciplinary Team Members, as well as the people supported and their families.
• Must be able to work independently, be organized, and possess the ability to prioritize work tasks.
• Must be at least 18 years of age

Physical Demands and Work Environment

• Must be able to regularly lift and carry up to 20 lbs. and occasionally pull at least 40 lbs.
• Must be able to squat, kneel, crawl, crouch, climb, and stoop as part of regular job duties.
• Must be able to use hands and fingers to handle or operate objects, tools, or controls.
• Required to stand and walk for extended periods.
• Must be able to demonstrate proficiency in CPR from floor level, which requires working on hands and knees, bending, standing, and lifting.
• Vision requirements include close, distance, and peripheral vision.
• Must be able to talk and hear.
• Exception – Deaf and Hard of Hearing Programs: For these programs only, employees must be fluent in American Sign Language (ASL). The requirement to talk and hear does not apply.
