Office Location: Remote within Trillium’s South Central Region, covering North Carolina counties: Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, and Wayne.

What We’re Looking For
Trillium Health Resources seeks an Intellectual/Developmental Disability (IDD) Monitoring Coordinator to join the Care Management team in the South Central Region. This role monitors Home and Community Based Services (HCBS) for Residential, Day Support, and Supported Employment, ensuring compliance with HCBS requirements during the member’s plan year and alignment with the final HCBS rule. The position requires a proactive approach to assessment, monitoring, and collaboration with the Care Management team to ensure quality support and adherence to waiver requirements.

Education/Experience:
Additional Qualifications: If serving members with LTSS needs, must meet minimum QP requirements and have at least two years of LTSS/HCBS coordination or related care management experience.
Preferred Experience: Experience with Intellectual Developmental Disabilities (IDD) or Traumatic Brain Injury (TBI) populations is preferred.
License/Certification: Valid driver’s license required. May be accepted per above.
Location: Remote within Trillium’s South Central Region. Travel to office locations and within the catchment area may be required.
