Job summary

RHA Health Services is hiring a Licensed Practical Nurse (IDD LPN) for a day shift in Sparta, NJ. This full-time, office-based position supports group homes and requires routine visits to residential homes.

Compensation is $34/hr with a $3,000 sign-on bonus and a Monday–Friday schedule, 8:30am–4:30pm. The role emphasizes structured, steady-paced care outside hospital settings, with prior IDD and case management experience encouraged.

Qualifications

• Graduate of an accredited licensed nurse program.
• Current LPN license with no restrictions to practice nursing.
• Current CPR certification or eligible to certify.
• Possess credentials required by Federal or State law.
• Must communicate effectively with Interdisciplinary Team Members, and the people supported and their families.
• Must work independently, be organized, and prioritize work tasks.
• Must be at least 18 years of age.

Responsibilities

• Provide holistic nursing care to individuals with intellectual and developmental disabilities.
• Observe, assist with gathering health related data, and report the health needs to the RN.
• Coordinate preventative and routine medical care for people supported.
• Document nursing care provided.
• Assist physicians with on-site medical clinics where applicable.
• Provide health related trainings to unlicensed personnel according to state regulations.
• Conduct medication pass observations to ensure staff competency.
• Conduct medication closest assessments.
• Perform quality assurance checks to ensure unlicensed personnel perform health tasks as assigned.
• Communicate with physicians, pharmacies, and the Interdisciplinary Team.
• Advocate for the people supported and develop community supports to promote health.
