Job summary

City of Chesapeake Virginia is seeking a Clinician II in Intellectual/Developmental Disability Case Management Services to coordinate, communicate, and collaborate with individuals, families, service providers and agencies to support individuals with intellectual or developmental disabilities.

The role develops service plans, conducts assessments, and links clients to services and benefits, while maintaining records and ensuring compliance with licensure and Medicaid requirements.

Qualifications

• Requires a bachelor’s degree in social work, counseling, psychology, or closely related field or other degree as approved by the Virginia Department of Health Professions.
• A master’s degree in Social Work, Counseling, or Rehabilitation is required for some assigned programs.

Responsibilities

• Develop individual service plans and implement plans in compliance with State Licensure and Medicaid regulations and standards.
• Conduct thorough assessments, prepare well-written service plans, reports, and progress notes, and maintain individual records in the electronic medical records system.
• Link, locate, and monitor services, resources, and benefits, and make collateral contact with community partners to promote the implementation of the individual service plan.
• Coordinate services with other agencies and providers involved with the individual and maintain necessary records to document the provision of case management services.
• Monitor service delivery and conduct home and agency site visits to observe and evaluate individuals’ responses to their environment.

Education

Bachelor's degree in social work, counseling, psychology, or closely related field

Master’s degree in Social Work, Counseling, or Rehabilitation (required for some assigned programs)
