Qualifications

• Bachelor's degree in one of the approved disciplines.
• Minimum of 1 year post-graduate experience with IDD populations.
• Strong written and verbal communication skills.
• Proficiency with computers and electronic documentation systems.
• Valid NYS driver license or NYS ID.
• Ability to travel within assigned region.
• Weekend availability when needed.

Responsibilities

• Conduct person-centered assessments for adults with intellectual and developmental disabilities.
• Work in home and community settings across Long Island and Staten Island.
• Maintain accurate electronic documentation and records.
• Collaborate with individuals, families, caregivers, and professionals.
• Travel throughout the assigned region and meet scheduling needs.
