Job summary

Allegheny County Department of Human Services is seeking a Promising Practice Coordinator. The coordinator will educate on evidence-based practices and develop services based on the LifeCourse Framework.

The ideal candidate will hold a Master’s degree in social work and have experience working in MH/ID settings. The position requires flexibility and travel within the county.

Qualifications

• Excellent communication skills, both oral and written.
• Ability to establish and maintain effective working relationships with clients and providers.
• Knowledge of ID/Autism Eligibility criteria.

Responsibilities

• Facilitate meetings and assess needs of individuals with complex needs.
• Collaborate with treatment teams for community transitions.
• Manage ISP related to service definitions and authorizations.
