Job summary

Qualifications

• 1 year of acute inpatient hospital RN experience preferred

• Develops and updates Plans of Care with measurable outcomes
• Documents assessments, care plans and patient progress in the medical record
• Collaborates with Case Management and physicians to coordinate discharge planning
