Qualifications

• Demonstrates ability to apply Nursing Process and coordinate care across disciplines.

Responsibilities

• Provides comprehensive nursing care per the nursing process.
• Coordinates patient care and education for patient and family.
• Performs accurate nursing assessments at admission, shifts, and change in condition.
• Administers medications and IV therapies per policy; ensures proper orders and monitoring.
• Documents on all care plans, assessments, and medications.
• Participates in discharge planning with Case Manager and physician.
