Qualifications

• Minimum requirements include an RN license and nursing diploma/associate's degree.
• 2–5 years of nursing experience required.
• Safety-sensitive position with focus on patient care.

Responsibilities

• Identify and resolve gaps in care from hospitalization through discharge.
• Lead complex care plans with interprofessional collaboration.
• Promote evidence-based practice and guide practice changes.
• Analyze practice gaps to improve patient safety and quality.
• Provide direct patient care and serve as a clinical resource.
