Job summary

UH Cleveland Medical Center is seeking an ICU Float Clinical Pharmacy Specialist to provide dedicated clinical pharmacy support across multiple adult ICUs. You will work with multidisciplinary teams to optimize therapy, monitor high-risk regimens, and contribute to safety and quality initiatives.

Qualifications

• PharmD degree and Ohio licensure.
• Experience in critical care pharmacotherapy and ICU settings.
• Ability to participate in teaching, research, and scholarly activities.

Responsibilities

• Participate in multidisciplinary rounds across adult ICUs.
• Provide evidence-based pharmacotherapy recommendations for critical care patients.
• Perform therapeutic drug monitoring and pharmacokinetic consultations for complex regimens.
• Support medication safety, safe use of formulary medications, and quality improvement initiatives.
• Contribute to the development and maintenance of critical care guidelines, protocols, and order sets.
• Document clinical interventions and outcomes; educate pharmacists, physicians, and residents.
