Job summary

HCA Houston Healthcare Conroe is seeking a Registered Nurse to coordinate and deliver high quality, patient-centered care in collaboration with medical providers and the care team. The RN will advocate for patients and families while modeling the organization’s vision and values to support outstanding outcomes.

You will perform procedures, educate patients and families, and contribute to continuous quality improvement within a supportive hospital setting in Conroe, Texas.

Qualifications

• RN license and/or eligibility
• Associate Degree or Bachelors Degree must be obtained within 30 days of employment start date
• No travel required
• No experience required (entry level possibilities)

Responsibilities

• Performs procedures, monitoring, or other functions as ordered by the medical provider(s). Documents the administration of care in the patient medical record in a timely and thorough manner.
• Teaches patients/families/caregivers about patient medical condition, status, treatment plan, medications and possible side effects, and follow-up measures; translates complex medical terminology for understanding.
• Teaches patients/families/caregivers about non-medicinal follow-up measures (diet, exercise, disease prevention) and prepares for future self-management.
• Manages evidence-based practices and continuous process improvement initiatives to meet and exceed quality measures and enhance patient care.
• Coordinates, evaluates, and delivers a holistic patient treatment plan in collaboration with a multidisciplinary team.
