Responsibilities

• Audit and abstract provider and/or vendor medical records for CMS code submission.
• Validate coding accuracy and documentation for risk adjustment processes.
• Apply HIPAA privacy and security requirements in handling patient data.
• Identify active vs historical conditions and ensure proper coding rationale.
• Meet strict timelines and perform self-audit checks for compliance.
• Communicate findings with providers and use industry standard tools.
