Coordinates multiple services and supports for youth with Serious Emotional Disturbance (SED). The care coordinator facilitates the development of a Care Planning Team, assists the family and Team in identifying goals and developing an Individual Care Plan and Safety Plan. Works directly with the youth and family to implement elements of the Plan, and coordinates with other Team Members involved with the family to help the youth and family reach their goals.

I. Major Responsibilities:

1. Demonstrates a knowledge of and practices the CBHI values : Child Centered and Family Driven, Strength-Based, Culturally Responsive, Collaborative, and Integrated, and Continuous Quality Improvement.

2. Maintains a mixed caseload of TM, IHT and ICC youth.

3. Conducts comprehensive home-based clinical assessments including the CANS; risk management, safety, and treatment/care planning.

4. Works with each youth/family to assemble a Care Planning Team composed of family members, their natural supports , and appropriate collaterals .

5. Coordinates and facilitates regular Care Planning Team meetings to develop individualized Care Pla ns for each enrolled youth/family.

6. Works closely with the FCT Family Partner, the family, and the Care Planning Team to implement, monitor and modify the Care Plan as needed, and serves as an advocate/liaison for the family when appropriate to access new or different services.

7. Collaborates and communicates regularly with other key service/clinical providers.

8. Effectively communicates and builds strong rapport with youth, families, colleagues, and/or community members in a manner that is sensitive to their racial/ethnic/cultural/class backgrounds.

9. Maintain client records based on role, including completing comprehensive assessments, CANS, care plans, treatment plans, intervention plans, safety plans, contact notes, and discharge summaries.

II. Position Qualifications:

License/Certification/Education:

Required:

1. Grade C06: Clinician II- Intensive Care Coordinator: requires minimum qualifications of Bachelor's degree in psychology, social work, or related human services field.
2. Grade C12: Clinician III: Intensive Care Coordinator: Master's degree in a clinical course of study in behavioral health (social work, psychology, marriage and family therapy, counseling or related field) that meet the academic requirements for independent licensure in psychology, social work, marriage and family therapy, or mental health counseling in the Commonwealth of Massachusetts. Requires licensure as LMHC, LCSW, LMFT, or is actively working towards licensure in the Commonwealth of
Massachusetts.
3. Grade C16: Clinician IV: Intensive Care Coordinator: Master's degree in a clinical course of study (social work, psychology, counseling or related field) and LICSW licensure or licensed Psychologist.

Experience/Skills:

Required:

1. Minimum of 1-year clinical experience with children, adolescents, and families with behavioral, social and/or emotional difficulties.

2. Proven ability to effectively engage youth and/or families, successfully advocate for clients and to collaborate well with other professionals .

3. Comfortable providing home-based and community-based services.

4. Demonstrated strong work ethic, ability to work independently and ability to contribute to team objectives .

5. Demonstrated effective communication, writing, and organizational skills.

6. A bility to use a computer-based information system.
