Job summary

PACS is seeking a compassionate Registered Nurse (RN) to join our team as a Hybrid Wound Care Nurse, delivering specialized wound care at both Westerville and Dublin skilled nursing facilities. The role involves assessing, treating, and documenting wounds, with travel between sites to ensure consistent, high-quality care.

The ideal candidate holds a Nursing Degree, has 2+ years of long-term care experience, and maintains CPR certification.

Qualifications

• Nursing degree from an accredited college or university.
• Two years or more experience in long-term care preferred.
• Current CPR certification.
• Must maintain nursing licensure in good standing.
• Ability to read and follow clinical/procedural documentation.

Responsibilities

• Assess, treat, monitor and document wounds.
• Collaborate with interdisciplinary teams to promote healing.
• Travel between Westerville and Dublin facilities for consistent care.
• Maintain accurate nurses' notes reflecting care and resident responses.
• Communicate plans of care with residents and families; explain treatments.
• Administer medications and treatments as prescribed.
• Assist in updating care plans and participating in training.
