Job summary

The Cuyahoga County Board of Developmental Disabilities (Cuyahoga DD) is seeking several full-time Support Administrators to facilitate person-centered plans for individuals, promote self-determination, and manage budgets and services.

You will act as the primary coordinator for planning teams, assess needs, identify funding, enroll in Medicaid waivers when needed, and ensure services are delivered per the ISP.

Qualifications

• Bachelor's Degree in Special Education, Social Work or Psychology required.
• Experience providing service coordination within a DD program recommended.
• Ability to obtain SSA certification within state timelines.
• Valid driver's license and insurable driving record required.

Responsibilities

• Coordinate person-centered planning using assess/plan/review/revise process.
• Develop, authorize and revise ISP budgets and funding sources.
• Monitor plan implementation and adjust services to meet outcomes.
• Collaborate with families, providers and natural supports to ensure coordinated services.
• Assist with Medicaid waivers enrollment and plan approvals.
